Avalon Place’s spacious rehabilitation extension is specially designed to provide the utmost quality and comfort during your short-term stay. Avalon Place’s Skilled Nursing & Rehabilitative Team strives to restore each resident as quickly as possible to his or her highest level of activity and independence.
Our therapists specialize in physical, occupational and speech therapy. Our approach provides the ideal solution for seniors who need help with daily activities, such as bathing, dressing, and medical supervision during their post-hospital stay.
While working daily with our full-time therapy staff, our residents enjoy beautifully appointed living spaces, delicious meals, engaging social activities, transportation, personalized assistance with care.
Avalon Place focuses on preparing our short-term stay residents back to independent living in their community.
A three day stay in the hospital and a diagnosis of any of the following will qualify you for our rehabilitation or skilled care unit:

Upon discharge from the hospital with orders for skilled care, he or she will be admitted to the rehabilitation or skilled care wing. Rehabilitation stays usually range from fourteen to one hundred days. An average length of stay is forty-seven days. During this time the patient’s room and board, medications, meals, x-rays, laboratory tests and physician visits are covered by Medicare and/or supplemental insurance. There are usually no out-of-pocket expenses.
Your loved one will receive up to three hours of therapy per day and twenty-four hour nursing supervision. The patient’s primary physician will follow the patient at Princeton Place. She or he must be seen once every thirty days.
Upon discharge back into the community, our therapists will do a home evaluation to ensure the transition is as smooth as possible. The rehabilitation team will evaluate your home and recommend to make living at home easier. We also can provide referrals to community services to ensure your at-home needs are served.
